REQUEST FOR PROPOSALS FOR CDBG GRANT ADMINISTRATION

CITY OF HIGH SPRINGS FOR FFY 2013

The City of High Springs hereby request proposals from qualified individuals or firms to provide Administration services for a Florida Small Cities Community Development Block Grant (CDBG) in the Neighborhood Revitalization category. The City has been awarded CDBG grant #15DB-OJ-03-11-02-N51 in the amount of $700,000.00 for the FFY 2013 funding cycle; therefore, procurement and contracting will follow CDBG regulations.

Administration services will include complete management and reporting for the project, with separate tracking for each funding source. A scope of work outlining the tasks to be performed and a proposed cost must be included in the proposal.

The evaluation criteria that will be utilized in the selection of a grant administrator are as follows:

               

1.The staffs number of years of experience with administering

projects through the State of Florida Small Cities

Community Development Block Grant (CDBG) program. 20 pts.

2. The number of years of experience of the firms management

group with the State of Florida Small Cities Community

Development Block Grant program. 20 pts.

3. Proposed approach to administration of the grant (requires

an outline of the proposed tasks to be performed).

20 pts.

4. Number of favorable client reference letters dated 2011 or

later provided from other communities. Please note, only

one (1) letter per community will be accepted. 20 pts.

5. The quality of the response from the client references

provided from other communities. 15 pts.

                                6. Fee or proposed fee basis.5 pts.

In the event of a tie, if one of the businesses involved in the tie is minority or female owned, they shall be ranked above the other firm or firms involved in the tie.

Proposals for CDBG Administration Services should include a fee for the services and an explanation or a basis for the fees proposed. Fees shall be lump sum for CDBG services.

Respondents are required to submit one (1) original and three (3) copies in a sealed envelope marked SEALED PROPOSAL FOR CDBG ADMINISTRATION SERVICES. Proposals must be received by 3:00 p.m. on Wednesday, November 12, 2014 at the City of High Springs City Hall Attention: Ed Booth, City Manager, City of High Springs, 110 NW 1st Avenue, High Springs, Florida 32643. Proposals will be publically opened on Wednesday, November 12, 2014 at 3:00 p.m. at the City of High Springs City Hall.

The City of High Springs reserves the right to reject any and all proposals, to waive any informalities or irregularities in the proposal process and to award the contract(s) in the best interest of the City.

Administration service contract(s) may be subject to grant/loan award and release of funds by the funding agency.

RESOLUTION 2014-11         BEFORE THE TOWN COMMISSION

A resolution of the Town of Micanopy, Florida, Recording a special assessment lien on real estate located at 201 & 203 Eestalustee Avenue (NW 6th Avenue Parcel #16520-031-001) for reimbursement of the costs of house demolition in the amount of $_________ incurred by the Town of Micanopy; Official owner of record is Ollie Mae Hooks and Leroy Miller; Providing for an Effective Date.

WHEREAS, the property located at 201 & 203 Eestalustee Avenue was determined to constitute a hazard pursuant to Chapters 5 and 20 of the Micanopy Code of Ordinances and found to be derelict and unsalvageable as a result of neglect. Specifically, the condition of the property violated Chapter 5, Article II, Section 5-20(a) and Section 5-20(b); Chapter 20, Article I, Section 20-2 and Section 20-3; Chapter 20, Article II Section 20-22 and Section 20-24 of the Micanopy Code of Ordinances; and

WHEREAS, the owner of record is Ollie Mae Hooks and Leroy Miller, 1522 SE 38th Court, Gainesville, FL 32641-9101; and

WHEREAS, notice was sent on December 5, 2013 requiring that corrective action be taken within ninety (90) days; and

WHEREAS, failure to correct these conditions within the allotted time resulted in the Town of Micanopy performing demolition work at a cost to the town in the amount of $3,278.70; and

WHEREAS, notification was made by process server on July 9, 2014 regarding the demolition costs and outstanding payment; and

WHEREAS, Section 5-27 of the Micanopy Code of Ordinances, set forth procedures for the Town of Micanopy to seek a special assessment against the properties located at 201 & 203 NW Eestalustee Avenue (NW 6th Avenue - parcel #16520-031-001) because of failure to reimburse the Town for demolition costs totaling $3,278.70; and

WHEREAS, per Micanopy Code of Ordinances Section 5-27 a public hearing was held on October 14, 2014 at which the Town Commission decided to assess a lien.

NOW THEREFORE BE IT RESOLVED by the Town Commission of the Town of Micanopy, Florida, that:

1.The Town Commission finds that:

A.The procedures of Article 5 of the Micanopy Code of Ordinances have been followed,

B.That the work done was in conformity with the requirements of Article 5 of the Micanopy Code of Ordinances,

C.That the following costs have been and will be incurred by the Town: (i) service of papers upon the owner in the amount of _____; (ii) publication costs in the amount of _________; (iii) recording costs in the amount of $18.50, and (iv) demolition costs in the amount of $3,278.70.

D.That an assessment of ________________, is just and reasonable and based on the actual costs of these proceedings.

2.An assessment in the amount of _____ is hereby levied against the property legally described as:

The North of Lot 31, Smiths Addition to the Town of Micanopy, Florida, as per Plat Book A, page 31 of the Public Records of Alachua County, Florida, less the Northwest of said Lot 31.

3.The assessment shall bear interest at the rate of six percent (6%) per annum on unpaid balances.

4.The lien of this assessment shall be of equal dignity with other municipal liens for taxes, levies, and assessments and may be enforced as other such municipal liens.

5.The owner may pay the amount of such lien, including interest, in thirty (30) consecutive monthly installments, commencing on the first day of the month following the adoption of such resolution.

6.A copy of this resolution shall be served on the owner by certified mail within ten (10) days of the adoption date.

7.A copy of this resolution shall be published once a week for four (4) consecutive weeks in a newspaper of general circulation within the Town.

8.A certified copy of this resolution shall be recorded in the public records.

8.This Resolution 2014-11 shall take effect immediately upon its adoption.

REQUEST FOR PROPOSALS FOR CDBG GRANT ADMINISTRATION

CITY OF HIGH SPRINGS FOR FFY 2013

The City of High Springs hereby request proposals from qualified individuals or firms to provide Administration services for a Florida Small Cities Community Development Block Grant (CDBG) in the Neighborhood Revitalization category. The City has been awarded CDBG grant #15DB-OJ-03-11-02-N51 in the amount of $700,000.00 for the FFY 2013 funding cycle; therefore, procurement and contracting will follow CDBG regulations.

Administration services will include complete management and reporting for the project, with separate tracking for each funding source. A scope of work outlining the tasks to be performed and a proposed cost must be included in the proposal.

The evaluation criteria that will be utilized in the selection of a grant administrator are as follows:

               

1.The staffs number of years of experience with administering

projects through the State of Florida Small Cities

Community Development Block Grant (CDBG) program. 20 pts.

2. The number of years of experience of the firms management

group with the State of Florida Small Cities Community

Development Block Grant program. 20 pts.

3. Proposed approach to administration of the grant (requires

an outline of the proposed tasks to be performed).

20 pts.

4. Number of favorable client reference letters dated 2011 or

later provided from other communities. Please note, only

one (1) letter per community will be accepted. 20 pts.

5. The quality of the response from the client references

provided from other communities. 15 pts.

                                6. Fee or proposed fee basis.5 pts.

In the event of a tie, if one of the businesses involved in the tie is minority or female owned, they shall be ranked above the other firm or firms involved in the tie.

Proposals for CDBG Administration Services should include a fee for the services and an explanation or a basis for the fees proposed. Fees shall be lump sum for CDBG services.

Respondents are required to submit one (1) original and three (3) copies in a sealed envelope marked SEALED PROPOSAL FOR CDBG ADMINISTRATION SERVICES. Proposals must be received by 3:00 p.m. on Wednesday, November 12, 2014 at the City of High Springs City Hall Attention: Ed Booth, City Manager, City of High Springs, 110 NW 1st Avenue, High Springs, Florida 32643. Proposals will be publically opened on Wednesday, November 12, 2014 at 3:00 p.m. at the City of High Springs City Hall.

The City of High Springs reserves the right to reject any and all proposals, to waive any informalities or irregularities in the proposal process and to award the contract(s) in the best interest of the City.

RESOLUTION 2014-11         BEFORE THE TOWN COMMISSION

A resolution of the Town of Micanopy, Florida, Recording a special assessment lien on real estate located at 201 & 203 Eestalustee Avenue (NW 6th Avenue Parcel #16520-031-001) for reimbursement of the costs of house demolition in the amount of $_________ incurred by the Town of Micanopy; Official owner of record is Ollie Mae Hooks and Leroy Miller; Providing for an Effective Date.

WHEREAS, notification was made by process server on July 9, 2014 regarding the demolition costs and outstanding payment; and

WHEREAS, Section 5-27 of the Micanopy Code of Ordinances, set forth procedures for the Town of Micanopy to seek a special assessment against the properties located at 201 & 203 NW Eestalustee Avenue (NW 6th Avenue - parcel #16520-031-001) because of failure to reimburse the Town for demolition costs totaling $3,278.70; and

WHEREAS, per Micanopy Code of Ordinances Section 5-27 a public hearing was held on October 14, 2014 at which the Town Commission decided to assess a lien.

NOW THEREFORE BE IT RESOLVED by the Town Commission of the Town of Micanopy, Florida, that:

1.The Town Commission finds that:

A.The procedures of Article 5 of the Micanopy Code of Ordinances have been followed,

B.That the work done was in conformity with the requirements of Article 5 of the Micanopy Code of Ordinances,

C.That the following costs have been and will be incurred by the Town: (i) service of papers upon the owner in the amount of _____; (ii) publication costs in the amount of _________; (iii) recording costs in the amount of $18.50, and (iv) demolition costs in the amount of $3,278.70.

D.That an assessment of ________________, is just and reasonable and based on the actual costs of these proceedings.

2.An assessment in the amount of _____ is hereby levied against the property legally described as:

5.The owner may pay the amount of such lien, including interest, in thirty (30) consecutive monthly installments, commencing on the first day of the month following the adoption of such resolution.

6.A copy of this resolution shall be served on the owner by certified mail within ten (10) days of the adoption date.

7.A copy of this resolution shall be published once a week for four (4) consecutive weeks in a newspaper of general circulation within the Town.

8.A certified copy of this resolution shall be recorded in the public records.

8.This Resolution 2014-11 shall take effect immediately upon its adoption.
